First of all, thank you for your interest in S.F.E.A.
Once you decide to become a member, we advise you to read our constitution. The S.F.E.A. membership will be granted only to personnel who have served in SHAPE or other NATO organizations as refered to in the constitution.
The S.F.E.A Membership will cover the period from 1 January to 31 December. The annual membership fee is €25,00.
How to start the proccess?
Download each application form below, compelete the details required by typing with a computer or typewriter and sign.
Do not forget to prepare a spare SF94 also for your spouse/legal partner.

How to pay the membership fee?
Once your documents approved, you will be requested to make your payment [€25,00] to our bank account: BE31 0016 0040 8555
by mentioning [YourName - 2025 Membership Fee] in the payment communication.
Where to send the application form?
Send (or bring) the original documents to President Luigi TESTA’s office at address:
Rue Potomac, Bldg. 250, Insurance Office, B7010, SHAPE MONS.
Finalization
Once we receive all the requested documents and you have been approved, you will be notified by e-mail and invited to S.H.A.P.E. for the final proccess of picture taking and receiving your S.H.A.P.E. Entry Pass. (Local residents only)
